Pericardial effusion simulating aortic dissection.

چکیده
The rapid widening of the mediastinal shadow is a very common radiological sign of acute aortic dissection1 2 and can suggest this condition in the proper clinical setting.3 In the presence of pericardial effusion, "bottle shape" configuration of the cardiac silhouette,4 straightening ofits contours,5 and acute cardiophrenic angles5 6 have been emphasised. Broadening of the mediastinal shadow has also been mentioned.46 We describe two examples of pericardial effusion, whose radiographic appearance and clinical presentation simulated acute aortic dissection.

Congenital pericardial defect with ruptured acute type A aortic dissection.
We report 2 cases of congenital pericardial defect with ruptured acute type A aortic dissection. Case 1: An 83-year old man presented with sudden chest and back pain, and computed tomography (CT) showed acute aortic dissection with left pleural massive effusion. Because of his unstable haemodynamic condition with low blood pressure, an emergency operation was performed. We observed small amount...
متن کاملGuide Catheter-Induced Aortic Dissection Complicated by Pericardial Effusion with Pulsus Paradoxus: A Case Report of Successful Medical Management
Aortic dissection is a rare but potentially fatal complication of percutaneous coronary intervention (PCI). Management strategies of PCI induced dissection are not clearly identified in literature; such occurrences often mandate surgical repair of the aortic root with reimplantation of the coronary arteries. Another trend seen in case reports is the use of coronary-aortic stenting if such lesio...
متن کاملPericardial Effusion in a Patient with Non-ST-Elevation Myocardial Infarction: Beware of a Hidden Malefactor
Pericarditis with pericardial effusion in acute coronary syndrome is seen in patients with ST-elevation myocardial infarction specifically when infarction is anterior, extensive, and Q wave. It is very uncommon to have pericardial effusion in a patient with non-ST-elevation myocardial infarction. We present an elderly hypertensive patient who was diagnosed as non-ST-elevation myocardial infarct...
متن کاملA Case Report of Type B Acute Aortic Dissection with Massive Pericardial Effusion
A 73-year-old man was transferred to our emergency room with a sudden onset of severe back pain, being under mechanical ventilation because of an intractable pain. His heart rate was regular and blood pressure was 166/96mmHg. A chest computed tomographic scan revealed a massive pericardial effusion associated with type B aortic dissection. His family told us that he had a history of hypothyroid...

Ivana JOVANOVIĆ Clinic for Cardiology, Clinical Center of Serbia, Višegradska 26, 11000 Belgrade, Serbia [email protected] SUMMARY Introduction Pericardial effusion can be a consequence of a number of pathological conditions, and as such it can cause impaired left ventricular filling followed by decreased cardiac output and blood pressure. This kind of hemodynamic compromise and its consequ...
